Appointments
All office consultations and visits are scheduled by appointment with the receptionist to insure best use of your time and ours. We request that you call in advance so that we can reserve a time for you. New patients will be encouraged to briefly describe the nature of your medical problem to the receptionist so that adequate time may be allowed for your appointment. Details of your insurance coverage will also be required to include forms, insurance card and information regarding pre-authorization and/or second opinion. We know that your time is as valuable as ours and we will make every effort to be on time but emergencies may delay us.
Online Forms
To aid in making your initial visit faster, online versions of all required forms are available. Forms listed in Patient Forms may be printed out, completed and brought to the office. If you have difficulty with, or are unable to use the online forms, all forms are available upon visiting our office.
