Instructions:
Download >> Click on the links to the left, when asked if you want to download the file or open file from it's current location, choose download file. Save the file to a place on your computer where you can easily find it.

Open Application >> After downloading and saving file, open Adobe Acrobat Reader. Choose File => Open. Search for the downloaded file and open it.

Printing File >> Once file is open, choose File => Print from the menu. Now all you need to do is fill out the printed forms and return them to Dr. Arnold's office.

NOTE: Some installs of Adobe Acrobat Reader, particularly on Windows based computers running Internet Explorer, will launch an Arcobat Web browser plug-in. In this instance, after clicking on the links to the left Adobe Acrobat Reader will load itself with in Microsoft Internet Explorer and also load the form. At this point simply follow the printing instructions.
