Instructions:
Download >> Click on the links to the left, when asked if you want to download the file or open file from it's current location, choose download file. Save the file to a place on your computer where you can easily find it.

Open Application >> After downloading and saving file, open your word processing program. For most users this will be Microsoft Word, Microsoft Write or Corel WordPerfect. It is recommended that you do not use WordPad to open the file. Once your word processing application is open, choose File => Open. Search for the downloaded file and open it.

Printing File >> Once file is open, choose File => Print from the menu. Now all you need to do is fill out the printed forms and return them to Dr. Arnold's Office.
